Prosper Healthcare and Consulting is pleased to partner with the Guilford Community Care Network (GCCN) to assure our community has access to the health care it needs.
GCCN is a network of doctors, dentists, healthcare professionals, pharmacies, and other agencies who volunteer their services to eligible low income and uninsured adults in
Guilford County.

The Orange Card offers:
- An assigned medical home (primary care provider)
- Referrals if you need to see a specialist or a dentist
- Care manager to help with community resources
- Healthy food from The Corner Farmer’s Market and Greensboro Farmer’s Markets
- Transportation to medical and pharmacy appointments
- Fill out the virtual application or print and return a paper application by mail.
